HIPAA Notice of Privacy Practices

Licensed providers working with us are required to maintain the privacy of your protected health information (PHI), give you this notice of their legal duties and privacy practices, and follow the terms of the notice currently in effect.

We use your PHI to review your records, conduct your evaluation, prepare your medical opinion, arrange payment for services, and run our operations. We disclose PHI to third parties — including an accredited representative, attorney or the VA — only with your written authorization, except where the law requires or permits disclosure without it.

You have the right to inspect and copy your records, request an amendment, request an accounting of certain disclosures, request restrictions on use or disclosure, request confidential communications, receive a paper copy of this notice, and revoke an authorization in writing at any time (except where we have already acted on it).

Uploads and storage use encryption in transit and at rest, access is role-limited to staff and providers working on your file, and access is logged. Vendors handling PHI on our behalf operate under business associate agreements.

If you believe your privacy rights were violated, contact us using the details below. You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights. We will never retaliate against you for filing a complaint.
